In order for us to process your request to charge your credit card, the Credit Card Authorization Form MUST be filled out

· The Credit Card Authorization Form MUST be filled out in full, signed, dated and either faxed to
   (1-970-797-1420) or E-Mailed to: orders@collateralmanagement.org.

Upon our receipt of the signed Credit Card Authorization Form, your appraisal order will be assigned to one of our local inspectors. They will contact you directly to set up your appointment.
